Primary Health-SMMPP, L.C

The business associates (BA), Primary Health-SMMPP, L.C., reported that an unauthorized actor accessed its server via SQL injection attack, affecting the protected health information (PHI) of 67,567 individuals. The PHI involved included clinical and demographic information. The BA provided breach notification to affected individuals, the media, HHS, and posted substitute breach notification online. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect its PHI.
