Premier Gastroenterology Specialists

The covered entity (CE), Premier Gastroenterology Specialists, reported that it sent an informational mailing to its patients which included the protected health information (PHI) of other patients. This breach affected approximately 1,260 individuals. The CE notified HHS, the affected individuals, the media, and established a toll-free telephone number for patients with questions or concerns. In its mitigation efforts, the CE implemented additional administrative safeguards to better protect its PHI and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
