The covered entity (CE), Planned Parenthood of the Heartland, reported that a box of files containing the protected health information (PHI) of 515 individuals was left unsecured in a building. The PHI involved included names, dates of birth, addresses, diagnoses, lab results, and other treatment information. The CE notified HHS and the affected individuals of the breach. In its mitigation efforts, the CE sanctioned the workforce member involved in the breach and retrained its workforce members to better protect PHI.
