The covered entity (CE), Piedmont Health Services, reported that its business associate experienced a ransomware attack that compromised the electronic protected health information (ePHI) of 674 individuals. The ePHI involved included names, addresses, dates of birth and death, drivers’ license numbers, Social Security numbers, diagnoses, and billing information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and increased the frequency of its security training and risk management processes.
