Piedmont Cancer Institute

The covered entity (CE), Piedmont Cancer Institute, P.C., reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 5,226 individuals. The ePHI involved included names, dates of birth, Social Security numbers, diagnoses/conditions, and financial information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. The CE also provided complimentary credit monitoring services. In response to the breach, the CE strengthened its administrative, technical, and security safeguards to better protect its sensitive data. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule.
