The business associate (BA), Physician Insurance A Mutual Company, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 1,852 individuals. The PHI involved included names, Social Security numbers, dates of birth, health insurance information, and other treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach the BA provided complimentary credit monitoring services and implemented new administrative and technical safeguards.
