Phoenixville Hospital

The covered entity (CE), Phoenixville Hospital, reported that an employee impermissibly accessed the protected health information (PHI) of 934 individuals. The PHI involved included names, addresses, dates of birth, financial information, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE sanctioned the responsible employee, provided complimentary credit monitoring services to those affected, and implemented additional administrative safeguards to better protect its PHI.The CE also created a call center for questions or concerns.
