Philadelphia FIGHT Community Health Centers

The covered entity (CE), Philadelphia FIGHT Community Health Centers, reported that it was the victim of a cyber-attack that compromised the protected health information (PHI) of 17,992 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards, revised its policies and procedures, and provided training to its employees.
