The covered entity (CE), Pershing Health System, reported that several employees were the subjects of an email phishing incident that compromised the protected health information (PHI) of 5,368 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE offered free credit monitoring services and implemented additional administrative, technical, and security safeguards. The CE also retrained staff on email security precautions.
