The covered entity (CE), People Incorporated, reported that several employees were the victims of an email phishing attack affecting the protected health information (PHI) of 27,500 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, the media, provided complimentary credit monitoring services. In response to the breach, the CE implemented additional technical safeguards and retrained its employees on email security.
