Patterson Dental Supply/Patterson Companies

An unknown individual hacked into the covered entity’s (CE) server which contained the electronic protected health information (ePHI) of approximately 6,400 individuals. The ePHI involved in the breach included names, addresses, dates of birth, social security numbers, payment information, and treatment information. The CE provided breach notification to HHS, affected individuals, and the media. Following the breach, the CE improved safeguards by installing a new firewall and filtering technology. Additionally, OCR’s investigation resulted in the CE retraining its employees.
