Orlando VA Medical Center

The Orlando VA Medical Center, the covered entity (CE), reported that an employee emailed documents containing the protected health information (PHI) of 9,850 individuals to a personal email account. The PHI involved included names, addresses, telephone numbers, email addresses, Social Security numbers, and birthdates. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services to affected individuals and obtained a court order requiring the safeguarding of PHI.
