The covered entity (CE), Onyx Technology, reported that its test server was the subject of a cybersecurity incident which encrypted and compromised the protected health information (PHI) 96,814 individuals. The PHI involved included names, dates of birth, addresses, phone numbers, and health insurance and other treatment information. The CE notified HHS and affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
