The covered entity (CE), One Medical, Inc., reported that an employee had inappropriately acquired the protected health information (PHI) of 1,009 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE sanctioned the employee responsible and provided complimentary credit monitoring services to affected individuals. Additional physical safeguards were also implemented.
