OAA Orthopaedic Associates

The covered entity (CE), OAA Orthopaedic Associates, reported that it was the victim of a malware attack affecting the protected health information (PHI) of 18,052 individuals. The PHI involved included names, addresses, dates of birth, claims information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards, revised its policies and procedures, and retrained its employees.
