The covered entity (CE), NYU Langone Hospitals, reported that an affiliated practice mailed the protected health information (PHI) of 1,141 individuals to the wrong recipients. The PHI contained in the letters was limited to the name of the intended recipient. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. As a result of the incident, the affiliated practice strengthened its administrative safeguards to better protect PHI.
