NLB Corporation, the covered entity (CE), reported that it experienced a cyber-attack affecting the protected health information (PHI) of 1,943 individuals. The PHI involved included names, dates of birth, Social Security and drivers’ license numbers, financial information, lab results, medications, passport numbers, health insurance information, and computer credentials. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring service and strengthened its administrative, technical, and security safeguards. Staff were retrained on the requirement to protect and secure PHI.
