The covered entity (CE), Nippon Life Insurance Company of America, reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 4,109 individuals. The PHI involved included names, Social Security numbers, diagnoses, and other treatment information The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
