Niagara County Department of Mental Health and Substance Abuse

The covered entity (CE), Niagara County Department of Mental Health and Substance Abuse, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 17,000 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services to affected individuals and implemented additional technical safeguards.
