The covered entity (CE), NewYork-Presbyterian Hospital, reported that its use of web tracking technologies on hospital websites may have resulted in a breach affecting the protected health information (PHI) of 54,396 individuals. The PHI involved included demographic, clinical and treatment information as well as IP addresses. The CE notified individuals, HHS, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative safeguards.
