The covered entity (CE), New York City Health and Hospitals Corporation - Metropolitan Hospital Center, reported that an employee sent emails that contained the electronic protected health information (ePHI) of 3,957 individuals, to his personal email account. The ePHI involved included names, dates of birth, and clinical and treatment information. The CE notified HHS, affected individuals, and the media. Following the discovery of the breach, the CE sanctioned the responsible employee and strengthened its administrative and technical safeguards.
