New York City Health and Hospitals Corporation - Harlem Hospital

The covered entity (CE), New York City Health and Hospitals Corporation - Harlem Hospital, reported that an unencrypted laptop containing the electronic protected health information (ePHI) of 595 individuals went missing. The ePHI involved included names, dates of birth, and clinical and treatment information. The CE notified HHS, affected individuals, the media, and reported the theft to the New York City Police Department. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI and provided security awareness training for workforce members.
