New Partners dba VNS Health Personal Care, the covered entity (CE), reported that multiple employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 5,175 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, the affected individuals, the media, and law enforcement officials. In its mitigation efforts, the CE implemented additional technical, administrative, and security safeguards. The mitigation efforts also included workforce member sanctions, updates to its HIPAA privacy and security policies and procedures, and updates to its HIPAA training.
