The covered entity (CE), the New Jersey Department of Human Services, reported that it experienced a cyber-attack that affected the electronic protected health information (ePHI) of 2,300 individuals. The ePHI involved included names, dates of birth, program addresses, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary identity theft protection and credit monitoring services. In its mitigation efforts, the CE strengthened its technical safeguards.
