Nevada Orthopedic & Spine Center

The covered entity (CE), Nevada Orthopedic & Spine Center, reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 50,000 individuals. The ePHI involved included names, addresses, dates of birth, phone numbers, health insurance information, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE revised its security policies and procedures, implemented additional administrative, technical, and security safeguards to better protect its ePHI, and retrained its workforce members. OCR obtained assurances that the CE implemented the corrective actions noted.
