Neurology Center of Nevada

The covered entity (CE), Neurology Center of Nevada, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 11,657 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses, lab results, and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and revised its policies and procedures and security awareness training.
