Nebraska Medical Center

The covered entity (CE), The Nebraska Medical Center, reported that an employee impermissibly accessed the protected health information (PHI) of 1,311 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and established a call center for questions or concerns. In response to the breach, the CE sanctioned the employee and strengthened its administrative safeguards.
