The covered entity (CE), NCH Corporation Employee Benefits Plan, reported that an unauthorized party gained access to its network environment through a zero-day vulnerability in third party software, accessing the protected health information (PHI) of 3,098 individuals. The PHI involved included demographic information. The CE notified HHS, the affected individuals, the media, and provided substitute notice.
