The covered entity (CE), Native American Health Center, reported that that it experienced a ransomware incident that affected the protected health information (PHI) of 4,231 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on privacy and security. During the investigation, OCR provided the CE with technical assistance.
