The covered entity (CE), Munson Healthcare, reported that an employee mailed the protected health information (PHI) of 1,186 individuals to the wrong recipients. The PHI involved included clinical and financial information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
