The covered entity (CE), Multnomah County Health Department, reported that a former workforce member retained possession of a laptop that contained the protected health information (PHI) of 1,092 individuals. The PHI involved included names, dates of birth, other identifiers, claims information, clinical and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative and technical safeguards to better protect its PHI.
