The covered entity (CE), Multnomah County, reported that its building was burglarized, and file cabinets were accessed. This breach affected the protected health information (PHI) of 709 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, drivers’ license numbers, email addresses, health insurance information, and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE retrained its workforce members and implemented additional physical safeguards.
