Moveable Feast

Moveable Feast, the covered entity (CE), reported that documents containing the protected health information (PHI) of 568 individuals were inadvertently placed in a recycling bin which wind overturned and scattered its contents, some of which could not be retrieved. The PHI involved included names, Social Security numbers, and other demographic information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative safeguards to better protect PHI. All workforce members were retrained on protecting and securing PHI.
