The covered entity (CE), Moses Lake Community Health Center, reported that its business associate suffered a cyber-attack that affected the electronic protected health information (ePHI) of 1,190 individuals. The ePHI involved included names, dates of birth, medical record numbers, and medications prescribed. The CE notified HHS, affected individuals, and the media.
