The covered entity (CE), Montefiore Medical Center, reported that a USB drive belonging to an employee was stolen. The device contained the protected health information (PHI) of 1,332 individuals. The PHI involved included names, dates of birth, email addresses, medical record numbers, diagnoses, and other clinical information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and strengthened its administrative, technical, and security safeguards to better protect its sensitive data.
