Molecular Pathology Laboratory Network

The covered entity (CE), Molecular Pathology Laboratory Network, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 339,471 individuals. The PHI involved included names, dates of birth, addresses, gender, phone numbers, email addresses, Social Security numbers, drivers’ license numbers, financial information, diagnoses, lab results, medications, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring to affected individuals, revised policies and procedures, and implemented additional administrative, technical, and security safeguards.
