The covered entity (CE), Midwest Long Term Care Services dba Senior Scripts, reported that it experienced a hacking incident that compromised the protected health information (PHI) of 10,566 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, health insurance information, and medication information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring and identity protection services and implemented additional administrative, technical, and security safeguards. OCR provided technical assistance to the CE.
