The covered entity (CE), Mercy Health, reported that an employee impermissibly accessed the protected health information (PHI) of 11,187 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, lab results, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE sanctioned the employee and provided affected individuals with complimentary credit monitoring services and implemented additional technical safeguards.
