Mercer County Joint Township Community Hospital

The covered entity (CE), Mercer County Joint Township Community Hospital, reported that it was the target of a hacking attack that compromised the protected health information (PHI) of 88,541 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, claims and financial information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals , the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect PHI. OCR provided the CE with technical assistance.
