Medstar Health, the covered entity (CE), reported that several employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 183,079 individuals. The PHI involved included names, dates of birth, addresses, clinical and financial information, and other identifiers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided additional security awareness training to all staff members and implemented additional technical safeguards.
