MedQuest Pharmacy

The covered entity (CE), MedQuest Pharmacy, Inc., reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 39,447 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, diagnoses, health insurance and claims information, financial information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards. All staff were retrained. OCR provided technical assistance to the CE.
