Medi-Lynx Cardiac Monitoring

The covered entity (CE), Medi-Lynx Cardiac Monitoring, reported that an employee was the subject of an email phishing attack that affected the protected health information (PHI) of 1,841 individuals. The PHI involved included names, dates of birth, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and provided substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect sensitive data. However, before OCR could complete its investigation, the CE was purchased and is no longer a CE in which OCR has jurisdiction; the case was therefore closed.
