McLaren Medical Group, Mid-Michigan Physicians Imaging Center

The covered entity (CE), McLaren Medical Group, Mid-Michigan Physicians Imaging Center, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 106,008 individuals. The PHI involved included names, dates of birth, addresses, phone numbers, diagnoses, and Social Security numbers. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical and security safeguards to better protect PHI.
