McKenzie County Health System, Inc., the covered entity (CE), reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 8,345 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims and financial information, diagnoses, conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained.
