Maxwell Aesthetics

The covered entity (CE), Maxwell Aesthetics, reported that it was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 656 individuals. The ePHI involved included names, dates of birth, diagnoses, health insurance information, and treatment information. The CE notified HHS, affected individuals, and the media. The CE also established a toll-free number for questions or concerns and provided free credit monitoring services to affected individuals. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
