The business associate (BA), Maximus, Inc. Reported that a workforce member impermissibly accessed and disclosed the protected health information (PHI) of 4,955 individuals. The PHI involved included demographic and financial information. The BA notified HHS, the affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect its PHI.
