Master Equity Texas Limited Partnership

The covered entity (CE), Master Equity Texas Limited Partnership dba Emerging-Solutions, reported that an employee impermissibly sent an Excel file to a physician’s practice that contained the electronic protected health information (ePHI) of an additional 1,962 individuals that were not a part of the physician’s practice. The ePHI involved included names, addresses, and diagnoses. The CE notified HHS and affected individuals. In its mitigation efforts, the CE strengthened its administrative safeguards and retrained its staff.
