Mason Tenders’ District Council Welfare Fund, the covered entity (CE), reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 13,344 individuals. The PHI involved included names, Social Security numbers, diagnoses, health insurance information, medications, and other treatment information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect PHI.
