Maquoketa Community School District

The covered entity, Maquoketa Community School District, reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 743 individuals. The ePHI involved included names, dates of birth, health insurance information, claims information, and treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary identity and credit monitoring services to all affected individuals. In its mitigation efforts, the CE implemented additional technical and security safeguards to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
