The covered entity (CE), Management and Network Services, reported that multiple employees were the victims of an email phishing scheme that compromised the protected health information (PHI) of approximately 30,132 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, medications prescribed, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
